I hereby release The Lodge at Cadron Ridge and staff from any and all liabilities,
financial or otherwise, for injuries to myself, my pets, or any other property of mine which arise
in the absence of gross negligence.
I waive any and all claims against The Lodge at Cadron Ridge relating to the care,
control, health, and/or safety of my pet(s) arising during transport, drop-off, and/or stay.
If group play has been approved for my pet(s), I waive any risks associated with a socialized
environment while under the care of The Lodge at Cadron Ridge
I agree to assume all liabilities and responsibilities, financial or otherwise, for the behavior and
health of my pet. I agree to hold The Lodge at Cadron Ridge harmless from any claims
which arise in the absence of gross negligence.
I authorize The Lodge at Cadron Ridge to do whatever they deem necessary for the
safety, health, and/or well being of my pet. I agree for my pet to be taken to a veterinarian
deemed appropriate by staff, and in the event of an emergency, I would like my pet to be taken
to an emergency facility for care. I agree to assume responsibility for all charges incurred.
I agree to pay the rates for pet care provided and for additional services requested. I further
agree that all charges shall be due and payable upon the departure of the pet(s) and that the
pet(s) shall not leave The Lodge at Cadron Ridge until all charges have been paid in full.
I agree to provide all current vaccinations records that are required.
I certify that, to the best of my knowledge, my pet has not been exposed to any
contagious diseases within a thirty-day period prior to check-in.
I acknowledge that The Lodge at Cadron Ridge has the right to refuse service to any pet
at staff’s discretion.
